Model to follow:
1- Date: When did you pay?
2- Name: So that I can check if your payment provide any informations that allow me to find you, it may be a name. (Example David Snow = Dav*** Sn***)
3 - Provide your SteamID64. You can find it, with the help of https://steamid.io/ or in-game by right clicking on your nickname in the scoreboard.
